Welcome to our Online Review Process

You are also welcome into our showroom to see spas in person to get the full in-person purchasing experience, we are open 7 days per week from 9am to 6pm, located at 280 Teller St Suite#140, Corona CA, 92879

IMPORTANT: This form is for delivery review and order verification only. No payment will be collected at this time.

To ensure a safe and successful delivery, Spa Max will review your delivery access photos, measurements, spa placement location, and electrical information before approving your order.
 

Once submitted, a Spa Max representative will personally review your information and contact you by phone to confirm all details, answer any questions, and verify that delivery is possible.
 

After delivery requirements have been approved and all information has been confirmed, we can proceed with secure payment either by phone or through a secure payment link. Delivery scheduling will be completed after the review and payment process has been finalized.

DELIVERY ACCESS & ELECTRICAL ACKNOWLEDGMENT

I acknowledge that I have uploaded clear and accurate photos of my delivery access path, including any gates, walkways, stairs, slopes, turns, or other potential obstacles. I have personally measured the narrowest point of the delivery path and entered the correct width in inches.

I understand that Spa Max relies on the information I provide to determine whether delivery can be completed safely and successfully. If the photos, measurements, or information provided are inaccurate, incomplete, or misleading, delivery may not be possible upon arrival.

I understand that Spa Max does not perform electrical installation work. It is my responsibility to hire a qualified electrician and ensure that all required electrical work is completed in accordance with local codes and the spa manufacturer's requirements.

I understand that Spa Max may provide a courtesy hookup at the time of delivery when all required electrical connections have already been completed and made accessible at the spa location. Courtesy hookups are provided solely as a convenience and are not guaranteed.

I understand that if the required electrical work has not been completed by the delivery date, Spa Max may still deliver the spa if conditions allow, but Spa Max will not return at a later date to perform a courtesy hookup once the electrical work has been completed.

I understand that any future service visit, return trip, or additional labor requested after delivery may be subject to additional charges.

I understand that failed deliveries resulting from inaccurate information may be subject to a $250 return/travel fee and may require additional scheduling, labor, equipment, or delivery charges before a future delivery attempt can be made.
